I managed to come up with a first name and surname from the letters but it wasn't a player I'd ever heard of. Good old Wikipedia told me I was right though and also led me to what he was first to do. |  |

Wikipedia is incorrect on the date, it was actually the 18th September 1965. The ever-reliable ColUdata has it right.
