While you're on, what is meant by sorting out medical issues? Is any renegotiating required? Or does it actually mean sorting the medical problem, ie. fixing the guy's body?!

Pretty impossible to 'fix' in most cases. There would/could be a bit of re-negotiating involved but the first thing really is to consult a specialist on whatever the matter may be. The player would need further inspection then to diagnose any serious issues. A medical is very general usually without looking too much into detail.
